      <image:title>Day Procedures - D&amp;amp;C Hysteroscopy - This operation involves passing a thin telescope through the cervix and into the cavity of the uterus. The cervix is grasped during this process to stabilise the uterus and this grasp point on the cervix can be a source of spot bleeding for a few days after surgery.     It is extremely uncommon to have any significant complications however........  Among the potential, well recognised but infrequent, complications is perforation of the uterus.    The management of this can be to simply halt the procedure. It can entail in-hospital observation for 24 hours. It is often simpler to perform a Laparoscopy to confirm there is no ongoing bleeding inside the abdomen and then complete the planned operation if all is well.</image:title>

      <image:title>Major &amp; Laparoscopic Surgery - Hysterectomy - Total Laparoscopic Hysterectomy (TLH)  All of the surgery in this advanced laparoscopic procedure is performed through 4 small incisions in the abdomen.  There is a 5mm incision at the umbilicus and a 7-8mm incision in the midline further down towards the pubic bone. There are  2 x 5mm incisions on either side of the lower abdomen. All of the suturing is performed through these incisions and so this approach is able to replace the large incision needed for a Total Abdominal Hysterectomy.    I have been performing this operation for over 15 years and now teach other interested specialists the technique.</image:title>

      <image:title>Incontinence Procedures - Tension-Free Vaginal Tape - The tape places no tension on the urethra because there a gap is left between the tape and the urethra.</image:title>
      <image:caption>The serrated edge of the tape grips the para-urethral tissues and allows the tape to remain firmly in place without suturing. The tape is placed under a general anaesthetic and utilises 3 small incisions. The entire operation takes about 30 minutes and there is minimal discomfort.     Originally designed as a 'day case' procedure, I sometimes keep women overnight as occasionally they require pain relief or have difficulty voiding due to swelling.     Cost is always an issue, but fortunately the device (prolene mesh tape - 1cm wide, with needles attached at each end) is only $700 and fully rebateable from Private Health Funds.   For the uninsured patient this is still cost effective when compared with at least 5 days ($220 per day intermediate) in hospital for a traditional Colposuspension (Bladder Neck Suspension), assuming there are no problems.</image:caption>
